1. I am pleased my child will be attending Bardykes Farm Nursery School and am happy to entrust the care of the staff.
2. I understand that Bardykes Farm Nursery School promotes equality and inclusion and employs both male and female practitioners who will carry out the same role and responsibilities.
3. . I understand my child will have daily contact with big & small animals both inside & outside and I understand the risks involved and I am confident the correct policies and procedures are in place to negate these risks
4. I understand that my child will often be outdoors on the farm perhaps in satelllite settings eg. hub, zen den, farmhouse kitchen, backfield and river.
5. I will inform management in writing of changes to our personal information on registration document.
6. I understand that I am required to choose a minimum of 2 days per week based on my child's pattern of attendance
7. I agree to pay monthly fees in advance, on the 1st of every month, the full amount based on pattern of attendance detailed in my invoice and any changes thereafter.
8. I understand that the agreed payment schedule will not change due to holidays and that no refunds will be issued due to sickness or any absences.
9. I agree to give 1 month notice before terminating this contract, reducing or changing sessions.
10. I understand that fees received later than 1 week from the 1st will result in an additional charge of 10% of my monthly fee per week and must be paid prior to my child returning.
11. I understand that if fees are overdue by 4 weeks, my child's place at nursery may be at risk and offered to a child on waiting list and that I am to liaise with nursery manager any financial difficulties.
12. I understand that the nursery has a late collection policy and will be charged £15 per 15 minutes after 6pm.
13. I understand that fee changes are reviewed annually and communicated with plenty notice.
14. I understand how funding works at Bardykes and agree to pay for additional services, daily trips to the farm and special resources.
15. I agree to be solely/jointly liable for fees and that the nursery may contact my workplace/or partner, if failure to pay fees.

16. If nursery is forced to close, no refunds will be made in current month. Full fees will be due the next month. Then 50% of fees will be waived until reopening.
17. I understand that only prescription medicines will be administered at nursery and I understand that I will be asked for written consent.
18. I have read and understood the nursery's complaints and child protection policies and am aware that I can access these for reference at any time. - These can be found on the Private Parents section of the website
19. I understand that if the nursery has any Child Protection concerns eg neglect or abuse, they have an obligation to report to authorities to fully investigate without prior consent or informing myself at the discretion of the Child Protection Officer.
20. I will inform nursery if my child will be absent, disclosing all symptoms. I understand if I do not, nursery may call my second, third contact to check all is ok
21. if no contact can be made and my child is absent from nursery with no contact being made for 24 hours, I understand that nursery has a duty of care to inform Social Work
22. I agree to follow NHS and Public Health Scotland Exclusion Periods (found in Parents Pack)

27. I understand that I MUST label all my child's belongings as failure to do so could result in items getting lost and the nursery accepts no responsibility for any lost or misplaced items.
28. I will discourage my child from bringing toys and other items from home as this may cause unnecessary conflict/upset between peers. Any items brought into nursery are not the responsibility of the setting.
29. I understand my information will be shared with South Lanarkshire Council and the Local Authority I reside within

30. I understand that I will be required to complete a funding application form in order for Bardykes to apply for Government funding on my behalf.
31. I understand that if applying for blended funded placement for my child, I am required to undertake a minimum of 20 hours at Bardykes
32. I agree to apply suncream before drop off when required and provide a sun hat and that suncream will be reapplied by the nursery throughout the day.
33. I understand that I am required to provide my child with their own suits and wellies suitable for outdoors and suitable for all weathers
34. I understand that I must provide my child with a pair of indoor shoes/slippers which will be left in the nursery.
35. I understand that I will provide suitable items including thick blanket or cot sheet for my child daily if they are to sleep at nursery, both indoors and outdoors
35a. I understand that if they are not brought, I will be contacted to bring these items to the setting before my child’s sleep time.
36. I understand that I must provide my child with a packed lunch with an ice pack for perishable products and thermos flask for any heated items.
36a. I understand that it is my responsibility to communicate with team members as to whether the contents are freshly made or processed and its cooking and reheating history.
37. I understand my child will eat snack(s) and lunch outdoors in various satellite picnic area and am comfortable with this.
38. I understand nursery will supervise risky play, block play and real tools inside & outside the nursery and am comfortable with this and I am confident that the nursery has the correct policies and procedures in place to negate these risks.
39. I have read, understood and agree to follow all of the above statements.
